Long Term Assurance, LLC (“LTA,” “we,” “us,” or “our”) helps families navigate long-term care benefits, including VA benefits and Medicaid. This policy explains what information we collect, how we use it, and the choices you have.

Information we collect

We collect information you provide to us directly when you contact us, engage our services, or complete our intake forms. Depending on the services you request, this may include:

  • Your name and the name of the applicant or care recipient
  • Contact information, including phone number, email address, and mailing address
  • Information about care needs, household, finances, and benefits eligibility that is necessary to prepare and submit a benefits application on your behalf
  • Documents you provide to support an application

We collect this information because it is required to determine eligibility for and to prepare applications for VA and Medicaid benefits. We do not collect more than we need for that purpose.

How we use your information

We use the information you provide to:

  • Determine potential eligibility for benefits
  • Prepare, submit, and follow up on benefits applications
  • Communicate with you about your case, including document requests, signature requests, appointment scheduling, and status updates
  • Meet our legal, regulatory, and recordkeeping obligations

Text messages and phone contact

If you provide your phone number to us, you may receive text messages from Long Term Assurance about your own case — for example, notifications that a document is ready to sign, appointment reminders, and follow-ups on your application.

Consent. We send text messages only to people who have given us their phone number and agreed to be contacted. Clients provide this consent verbally when they give us their phone number in the course of engaging our services, after we explain that we send text updates about their case. We follow up in writing by email confirming that text updates have been enabled and how to stop them. We do not send marketing or promotional text messages, and we do not text people who are not our clients.

Opting out. You can stop receiving text messages at any time by replying STOP to any message. You will receive one confirmation message and no further texts. Reply START to resume. Reply HELP for assistance, or call us at 602-288-9094.

What opting out means for your case. We cannot advocate for you if we cannot reach you. Benefits applications depend on responding to agency requests, deadlines, and signature requirements, often on short notice. If you opt out of text messages, we may contact you by phone, email, or mail to explain what that means for your application and to ask whether you would like to restore text contact. We will not text you again unless you ask us to. If you decline, we will honor that and stop.

If you withdraw from communication with us altogether, we may be unable to continue representing you on your application, and fees already paid for work performed are not refundable. The terms of your engagement with Long Term Assurance govern in these circumstances.

Message frequency and rates. Message frequency varies based on the activity on your case. Message and data rates may apply, depending on your mobile plan.

We do not sell or share your phone number or your text messaging consent with third parties for their own marketing purposes. Your text messaging consent is used only by Long Term Assurance to communicate with you about your case, and is never transferred to anyone else. The one circumstance in which we give your name and phone number to another party is a referral you have personally agreed to — described under “How we share information” below — and that never includes your consent to receive texts from us.

How we share information

We share your information only as needed to do the work you have engaged us for, or where the law requires it:

  • With the agencies handling your claim, including the U.S. Department of Veterans Affairs and state Medicaid agencies, as part of preparing and submitting your application.
  • With service providers who support our operations, such as our case management, document storage, email, and messaging systems. These providers process information on our behalf, under agreements limiting them to that purpose. They are not permitted to use your information for their own marketing.
  • With professionals we refer you to, and only with your permission. Some situations call for help we do not provide ourselves — for example an attorney, a placement agent, or a Medicare agent. We ask for and receive your verbal confirmation before we pass your name and phone number to anyone. We share only your name and phone number for this purpose, we do not share the details of your case, and we never make a referral you have not agreed to. If you would rather contact the professional yourself, tell us and we will give you their information instead.
  • When required by law, such as in response to a subpoena, court order, or other legal obligation.

We do not sell your personal information. We do not share it with third parties for their own marketing purposes.

How long we keep information, and deleting your information

We keep client records for as long as needed to provide our services and to meet legal and regulatory recordkeeping requirements.

You may ask us to delete your information, and we will do so unless we are legally required to retain specific records. To make a request, contact us using the information below. We may need to verify your identity before acting on a request.

Security

We use administrative and technical safeguards to protect the information in our care, including restricting access to staff who need it to do their work. No method of storage or transmission is completely secure, and we cannot guarantee absolute security.

Minors

Our services are directed to adults. We do not knowingly collect personal information from anyone under 18.

Changes to this policy

We may update this policy from time to time. When we do, we will revise the “Last updated” date above.
